Position Responsibilities Conduct comprehensive assessments of patients’ physical, emotional, and spiritual needs. Create and implement personalized care plans in collaboration with a multidisciplinary healthcare team. Provide effective symptom management and palliative care interventions. Offer education and support to patients and families, including advanced care planning and end-of-life options. Coordinate care transitions, including discharge planning to home or assisted living environments. Perform clinical procedures including catheter care and botulinum toxin administration as needed. Maintain compliance with healthcare standards and palliative care best practices. Required Skills and Experience Strong clinical expertise in acute care, emergency medicine, and discharge planning. Demonstrated experience in palliative care, family practice, or geriatrics. Excellent communication and collaboration skills. Compassionate and empathetic approach focused on improving patients’ quality of life. Deep understanding of palliative care principles and symptom management. Basic Requirements Current NP license in North Carolina (or compact state). Basic Life Support (BLS) certification from AHA or Red Cross. One year of recent experience as a Nurse Practitioner in a relevant setting.
