NURSE PRACTITIONER/PHYSICIANASSISTANT
Job Description:
Provides client services to localSkilled Nursing & Rehabilitation facilities. Assures qualitynursing care for patients by directing and coordinating nursingcare. Conducts physicals and annual assessments for assignedpatients. Completes acute visits to assess and diagnose patientconditions. Focuses on early identification of changes in conditionas well as strong management of chronic conditions. Completeshistory and physical on new admissions within 72 hours of admissionor readmission. Completes Medicare certifications according torequired time frame. Prepares discharge orders, instructions andprescriptions for patients being discharged. Encourages advancedcare planning and end of life decisions for patients requiringthese services. Provides pharmacy with needed narcotic prescriptionrefills as patient need dictates. Documents patient assessment andplan of care in the electronic medical record. Reviews labs andother diagnostic tests for additional interventions. Collaborateswith physicians and staff. Notifies family of changes in condition.
Communicates daily with nursing staff, assigns duties, supervises,and evaluates work performance. Reviews nursing policies andprocedures for compliance, and to make recommendations forchanges/additions as necessary. Recommends or conducts specificareas of in-service or continuing education. Assesses patients forpalliative care needs and creates/oversees the palliative plan ofcare In facilities and in patient’s homes.
Job Requirements:
Current, unrestricted valid Nurse Practitioner license from theBoard of Nursing/PA License from the Medical Board in the state ofassignment or the ability to obtain. DEA certificate current or theability to obtain. Clinical background in adult, family orgeriatric specialties preferred. Past experience working inlong-term care, a nursing home, or with seniors in other settingspreferred. Strong communication and relationship building skillspreferred. Intermediate level of proficiency in PC based wordprocessing, including Microsoft Word and Outlook. Willing to be ‘oncall’ for Facilities as needed. Dependable, flexible, and able towork and cooperate well with all nursing personnel and otherdepartments.
