Practice Setting:

Outpatient setting
A majority of the patient population is aged 65+
Support staff consists of nurse practitioners, medical assistants, and registered nurses

• Schedule Details:

• Monday- Friday, 8 AM – 5 PM
  Flexibility for a 4-day workweek is available
  No call requirements
  Credentialing timeframe is approximately 2 weeks

• Cases & Procedures:

• Traditional primary care for older adults, including:
  Diabetes, Hypertension, Hyperlipidemia, Mild cognitive impairment, CHF, and a history of coronary artery disease
  No procedures performed

• Location:
