Nurse Practitioner for Long Term Care and Post-Acute Care M Medical Group, INC Garner, NC 27529 Medical Group, INC is seeking a Nurse Practitioner with prior post-acute/long-term/skilled nursing facility experience. Possess Board Certification in Geriatric, Family, or Adult Nurse Practitioner (ANCC, ACHPN, or AANP) Must have at least one year of experience in the post-acute/SNF/LTC. M-Medical Group, Inc., is one of the region’s fastest-growing post-acute care medical practices. M-Medical Group, Inc. seeks a Nurse Practitioner to add to its growing list of providers in the Garner, NC area and vicinity. The NP’s role is to provide high-quality, effective management of patients facing debilitating chronic diseases. Manage patients with acute and chronic diseases in collaboration with the Attending Physician. Communicate and collaborate with Director of Nursing/Administrator, Social Worker and the entire multidisciplinary team.
