Job Description

  • Located in Alton, 11m to Wolfeboro
  • Full Time Position

Job Summary:

Under the supervision of the Chief Medical Officer, this Advanced Practice Provider (APP) works collaboratively with physicians and other healthcare professionals to deliver high-quality care to patients in a primary care setting. The role involves working at two practice locations and supporting patients with professionalism, compassion, and clinical excellence.

Primary Responsibilities Direct Patient Care:

Conduct thorough physical exams, medical histories, and diagnostic evaluations Develop and implement individualized care plans Order and interpret diagnostic tests, prescribe medications, and manage referrals Address patient questions regarding medications, diet, and overall health Recognize urgent clinical situations and take appropriate action Communicate follow-up care and health guidance clearly and compassionately

Care Coordination:

Collaborate with a multidisciplinary team to ensure cohesive patient care Refer patients to internal care coordinators and appropriate specialists as needed Coordinate care with patients and their families, emphasizing clear communication Maintain open dialogue with referring providers to ensure continuity of care

Record Utilization:

Utilize the Electronic Medical Record (EMR) system to document patient care accurately and efficiently Record clinical findings, treatment plans, and patient responses in accordance with best practice workflows

Organizational Responsibilities:

Participate in staff meetings and support departmental objectives Maintain confidentiality across all aspects of clinical and organizational operations Pursue continuing education to stay current in clinical best practices

Organizational Expectations:

Demonstrate consistent attendance and punctuality Adhere to HIPAA and patient confidentiality standards Foster a culture of teamwork, respect, and service excellence Maintain safe workplace practices at all times

Employee Engagement:

Engage in service excellence trainings and improvement initiatives Attend at least 50% of departmental meetings and review all meeting minutes Complete all required annual education on time

Knowledge, Skills, and Abilities:

Ability to manage physical tasks including light lifting (up to 25 lbs.) Strong interpersonal, verbal, and written communication skills Ability to work under mental stress and handle urgent situations Proficiency in using computers and standard office/medical equipment

Qualifications:

Graduate of an accredited Nurse Practitioner or Physician Assistant program Active license to practice in the State of New Hampshire Applicable national certification per licensure Active and unrestricted DEA registration

Preferred:

Three or more years of experience as an APP
