NP (Nurse Practitioner) Family Medicine;Primary Care KbRani Jaffrey, NH 03452 Job Overview We are seeking a dedicated and compassionate Primary Care Nurse Practitioner to join our healthcare team. The ideal candidate will provide comprehensive care to patients, focusing on preventive health measures, chronic disease management, and overall wellness. This role requires a strong understanding of medical protocols and the ability to work collaboratively with other healthcare professionals to ensure the best patient outcomes. Responsibilities Conduct thorough patient assessments and develop individualized care plans. Diagnose and treat a variety of medical conditions, utilizing evidence-based practices. Perform medical procedures as necessary, including but not limited to sonography and medical imaging. Document patient interactions accurately in electronic health records (Epic) and maintain up-to-date medical documentation. Educate patients and their families about health management, preventive care, and treatment options. Collaborate with interdisciplinary teams to coordinate home care services and senior care initiatives. Stay current with ICD-10 coding practices for accurate billing and documentation. Participate in quality improvement initiatives to enhance patient care delivery. Requirements Master’s degree in Nursing from an accredited program. Current Nurse Practitioner certification with a valid state license. Experience in medical-surgical nursing or primary care preferred. Proficiency in medical documentation practices and familiarity with electronic health record systems, particularly Epic. Knowledge of gastroenterology principles is a plus. Strong communication skills and the ability to build rapport with patients of all ages. Commitment to providing high-quality patient-centered care.
Schedule:
Monday-Friday, 8a-5p
Call Rotation:
1:5 beeper call
Patient Census:
Up to 18 per day
Team Structure:
1 MD, 2 APPs currently
Preferred Experience:
5+ years Must be board-certified or board-eligible
