Job Description

We are seeking a skilled and compassionate Primary Care Nurse Practitioner or Physician Assistant to join our outpatient team in Colebrook, NH. This full-time role involves providing comprehensive, longitudinal care to patients across the lifespan—from pediatrics to geriatrics—with a focus on chronic disease management and preventative care. Clinicians are supported by a collaborative care team and work in a setting that values autonomy, clinical excellence, and strong provider-patient relationships.

Practice Highlights:

Outpatient-only schedule: 36 patient-facing hours + 4 administrative hours weekly Typical patient load: 16-22 patients per day No on-call responsibilities

Meditech Expanse with Dragon dictation Clinical support includes an RN Team Leader and Medical Assistant Extensive onsite specialty services including allergy/ENT, cardiology, dermatology, neurology, orthopedics, GI, OBGYN, pulmonary, rehab therapies, and more

Requirements:

Board certified as a Nurse Practitioner or Physician Assistant Minimum 2 years of experience in a Primary Care outpatient setting Current ACLS and BLS certifications through the American Heart Association Comfortable managing an independent patient panel and working autonomously Strong background in chronic disease management, particularly with elderly patients About the
