NP (Nurse Practitioner) Family Medicine;Primary Care KbRani Peterborough, NH 03458 Job Summary We are seeking a dedicated and compassionate Primary Care Nurse Practitioner to join our healthcare team. The ideal candidate will provide high-quality patient care across various age groups, including infants, while demonstrating expertise in medical-surgical nursing and telehealth practices. This role requires a strong understanding of psychopharmacology and the ability to utilize electronic health record systems such as Athenahealth for accurate medical documentation. Duties Conduct comprehensive health assessments and develop individualized care plans for patients. Diagnose and manage acute and chronic health conditions, ensuring evidence-based practices are followed. Provide telehealth consultations to enhance patient access to care. Administer medications and perform necessary procedures in accordance with established protocols. Maintain accurate medical documentation, including ICD-10 coding, to support patient care and billing processes. Collaborate with interdisciplinary teams to coordinate patient care and referrals as needed. Educate patients and families on health management, disease prevention, and wellness strategies. Stay current with advancements in healthcare practices, particularly in gastroenterology and infant care. Requirements Master’s degree in Nursing or equivalent qualification as a Nurse Practitioner. Current state licensure as a Nurse Practitioner with prescriptive authority. Proficiency in medical-surgical nursing practices and telehealth technologies. Strong knowledge of psychopharmacology principles relevant to patient care. Familiarity with Athenahealth or similar electronic health record systems for efficient documentation. Understanding of ICD-10 coding standards for accurate medical record keeping. Excellent communication skills, both verbal and written, with the ability to engage effectively with patients of all ages. Knowledge of physiology related to various health conditions is essential.
