Full-Time or Part-Time NJ Palliative Care is hiring an experienced APP to provide compassionate, high-quality palliative care to patients in homes, assisted living communities, and virtually across Jersey City. This position is ideal for a provider who values meaningful connections and delivering holistic care outside the hospital walls.

Your Impact:

Conduct in-home and telehealth visits for symptom management Create personalized care plans that align with patients’ goals Build trust with patients, families, and referring providers Facilitate advanced care planning and end-of-life discussions

Requirements:

NJ NP or PA license with DEA and CDS registration 3+ years of experience in palliative care, primary care, or home health Excellent assessment and communication skills Reliable transportation and ability to work independently

Compensation & Benefits:

Salary:
