harmonycares.com/benefits. Responsibilities TheNurse Practitioner delivers annual risk assessment in a residentialsetting or telehealth, within the scope of practice for a NursePractitioner, as delegated by the Collaborative Physician.
Essential Duties and Responsibilities
- Conduct comprehensivein-home health risk assessments to identify all active and chronicdisease conditions, as well as determine all physical, mental, andsocial needs present at the time of the visit
- Takes history,examines, determines diagnoses.
- Provides written documentation ofpatient visit, per NCQA standards
- Takes patient vital signs, asnecessary.
- Places case management referrals and communicates withPCP as necessary.
- Communicates with patients, caregivers, agencynurses, other providers and vendors as necessary to assure properdiagnosis.
- Performs all clinical duties while observing OSHAUniversal Precautions
- Maintains patient confidentiality
- Attendsrequired meetings and in-services and participates in committees,as requested

- Active/unrestricted medical license.
- Active CPR Certification
- Board eligible or board certified in family medicine.
- OutstandingEHR skills Preferred Knowledge, Skills and Experience
- Geriatrictraining/experience
- Skill in teamwork and maintaining effectiveworking relationships with patients, medical staff, and the publicConditions of this role to be aware of. . .
- Adaptability todiffering weather conditions and patients’ home/residentialenvironments
- Full range of body motion including handling/liftingpatients. Manual and finger dexterity, eye-hand coordination,normal visual acuity, normal hearing, standing, bending, walkingand stair climbing
- Regular lifting/carrying items weighing up to50 pounds
- Ability to ride in automobile or van up to 150 milesdaily in urban and/or rural settings.
Ability to drive, ifnecessary Posted Min Pay Rate USD $128,000.00/Yr. Posted Max PayRate USD $134,000.00/Yr. Pay Transparency Individual compensationpackages are based on various factors unique to each candidate,including skill set, experience, qualifications, and otherjob-related considerations.
