Essential Duties and ResponsibilitiesConduct comprehensive in-home health risk assessments to identifyall active and chronic disease conditions, as well as determine allphysical, mental, and social needs present at the time of the visitTakes history, examines, determines diagnoses. Provides writtendocumentation of patient visit, per NCQA standards Takes patientvital signs, as necessary. Places case management referrals andcommunicates with PCP as necessary. Communicates with patients,caregivers, agency nurses, other providers and vendors as necessaryto assure proper diagnosis. Performs all clinical duties whileobserving OSHA Universal Precautions Maintains patientconfidentiality Attends required meetings and in-services andparticipates in committees, as requested Participates inprofessional development activities and maintains professionallicenses and affiliations In this role you may work with. . .Teammates Physicians Medical Staff Patients Caregivers AgencyNurses Providers Vendors Qualifications Required Knowledge, Skills,and Experience Active/unrestricted medical license.
