Home Health Pay:
$175 per visit Responsibilities Conduct thorough patient assessments and develop individualized care plans. Provide comprehensive outpatient care, including diagnosis and treatment of various health conditions. Perform IV insertions and catheterizations as necessary for patient treatment. Manage infectious disease cases, ensuring appropriate follow-up and monitoring. Utilize Cerner for accurate documentation of patient information and care plans. Collaborate with multidisciplinary teams to coordinate patient care and case management. Educate patients and their families on health management, disease prevention, and wellness strategies. Stay current with advancements in physiology knowledge to enhance patient outcomes. Skills Proficient in IV insertion techniques and catheterization procedures. Strong understanding of infectious disease care protocols. Experience in outpatient settings with a focus on hospital medicine. Excellent case management skills to effectively coordinate patient care. Knowledge of Cerner or similar electronic health record systems is preferred. Ability to apply physiological knowledge in clinical decision-making processes. Strong communication skills for effective interaction with patients and healthcare teams.
