- 5:00PM As a NursePractitioner Supervisor, you will: The Nurse PractitionerSupervisor (NPS) will work in collaboration with contractedcommunity primary care physicians (CPCP)to ensure proper oversightand coordination of participant care with the InterdisciplinaryTeam.
The NPS will work with the community physician to coordinateand support the participant’s functional, clinical, andpsychosocial needs, with an emphasis on independence and wellness.
This role will work as the liaison between the participant, thecommunity physician, and the plan staff to develop and implementthe plan of care, including person-center goal development,communicating changes in the participant’s condition, andfacilitating primary care preventative services.
KeyResponsibilities:
Supervises and manages Nurse Practitionersassigned to their location and assists with the oversight of otherDiagnostic Treatment Center’s clinical staff as needed. Completedirect reports and annual performance appraisals and collaboratewith other DTC Managers providing annual performance feedback tothe DTC clinical staff as needed. In the absence of, or at thedirection of, the Medical Director, the NPS will represent or serveas the Medical Director role at their location. At a minimum,perform annual wellness assessments on all assigned siteparticipants and ensure reflects an understanding of the complexityand multiple comorbidities of the frail and/or elderly. Forparticipants with higher levels of risk, these assessments will beperformed more frequently to appropriately address his/her level ofneed, as described below. Provides urgent care services toparticipants and facilitates the provision of same-day access.
Assesses and manages participants with acute changes in conditionin a timely manner. Assesses participants upon return from thehospital/Emergency Department within five (5) days. This includesthe reconciliation of medications and treatment plans andcoordination with the participant’s CPPCP and IDT members to ensurethe timely transition of care follow-up visits. Reviews thepatient’s past medical history at least every six (6) months andformulate a comprehensive and complete diagnostic list of currentand past medical conditions using clinical knowledge and judgmentand the findings of their assessment. The NPS is responsible forensuring that all such documentation is complete and accurate, andspecific diagnosis codes will be documented in clinical platformand as encounter data as required. Participates in all clinicaldocumentation improvement activities with the goal being anaccurate and full capture of the burden of illness of ourparticipants and appropriate risk adjustment. Provide feedback andcounseling to DTC and IDT staff regarding their clinicaldocumentation as needed. Reviews the Participant’s current symptomsand exacerbation of problems that were previously controlled andidentifies active diagnoses and chronic problems or conditions tobe used in Care Management and active medical management oftreatment and designed interventions. Co-manages the highest risksubset of participants with the PCPs and communicates findings ofassessments to inform Participant’s PCP of potential gaps in careand coordinates with CPCP on a care plan that will address thesegaps. Attends and participates in Interdisciplinary team (IDT)meetings and represents CPCP when they are unavailable to attend ordelegates the role to the NP. Communicates with IDT team or otherPlan designees in accordance with s policies and procedures.
Participates in complex care management and educates participantsand their families and staff on such topics as disease processes,medication adherence, and self-management promotion. Complies withall HIPAA regulations and maintains security of Protected HealthInformation. Act as the direct supervisor of the care planformulation and the proper delivery of it by each discipline forthe community PCP participants; the care plan must be tailored tothe needs of the individual, and all interventions and outcomesmust be followed and supervised by the practitioner. Supervises thehome care services that the CHN arranges/provides for the communityPCP participants. Responsible for ensuring that the carecoordinated by the CHN is appropriate and deemed for the patient’sneeds, including but not limited to scheduling, plan of caredevelopment, and overseeing the clinical outcomes. On-call rotationwill be expected as part of the job duties. Only act within thescope of the individual’s authority to practice. Acting member ofthe IDT. Performs other duties as assigned.
The ideal candidatewill have:
Education:
Graduated from a nurse practitioner educationprogram acceptable to NYSED or certified as a nurse practitioner bya national certifying organization acceptable to NYSED.
Experience:
Minimum of two (2) years in clinical nursing practice in-home care,care management, or related field required. Minimum of one (1) yearof experience working with a frail or elderly population or, if theindividual has less than one (1) year of experience but meets allother requirements, must receive appropriate training from theorganization on working with a frail or elderly population uponhiring.
Licensed:
required to practice as a Nurse Practitioner inNY. DEA Certification required. AANP or ANCC Board Certificationrequired. Qualified candidates should
