PHYSICIAN
(FAMILY
PRACTICE
) Jericho Road Community Health Center is actively seeking a Physician (Family Practice) . This position is a full-time, within the Medical Care Department, working across all medical sites.
Responsibilities:
Maintain electronic medical records efficiently and maximize resources. Provide high quality primary care for adult, pediatric, and obstetric patients of Jericho Road Community Health Center. Offer comprehensive, patient- and family-centered care. Ensure consistent, cost-effective care. Provide inpatient adult, pediatric, and obstetric care as part of a call pool with other family medicine and OB/GYN physicians. Provide patient care in a wholistic manner including attention to physical, mental, emotional, and spiritual health. Collaborate with interdisciplinary clinical and program teams at Jericho Road Community Health Center. Create a learning plan that includes familiarity with new research and changes the standard of practice. Participate in quality improvement activities with a data-driven approach. Willingness to work with family medicine and pediatrics residents. Attending required staff and provider meetings except clinical activities prevents such attendance.
Qualifications:
Doctor of Medicine or Doctor of Osteopathy, licensed to practice in New York State prior to start date of employment Completion of three-year or four-year Family Medicine Residency Board-eligible from American Board of Family Medicine. Board-certified within one year of employment. Passion for high quality primary health care and community development among a culturally and linguistically diverse urban population Demonstrated ability to develop collaborative working relationships Comfortable working in a fast-paced, team environment Willingness to be flexible Proficient in utilizing electronic health records Reliable transportation within the city of Buffalo Experience working with low-income, diverse, multilingual populations highly desired
Hourly Rate:

ACUTE CARE MEDICAL PROVIDER
Jericho Road Community Health Center is actively seeking a Acute Care Medical Provider . This position is a full-time, within the Medical Care Department, working across all medical sites.
Responsibilities:
Improve patient outcomes and deliver quality care within practice areas as assigned. Provide high quality acute medical care for adult, pediatric, and basic gynecology patients of Jericho Road Community Health Center Follow up with patients regarding lab and imaging results Ensure all clinical care is delivered in a manner that meets or exceeds goals and expectations for clinical outcomes, quality assurance standards, state/local regulations, and patient satisfaction. Participate in an office call pool for patient phone calls overnight. Work one Saturday morning clinic per month. Provide patient care in a wholistic manner including attention to physical, mental, emotional, and spiritual health Maintain electronic medical records efficiently and maximize resources Create a learning plan that includes familiarity with new research and changes to standard of practice Maintains current certifications including all mandatory annual and CPR training Ability to adhere to professional code of ethics, and compliance to Article 28 and FQHC requirements Collaborate with interdisciplinary clinical and program teams at

(MD/DO, NP, PA) Jericho Road Community Health Center is actively seeking a Homeless Care Medical Provider . This position is full-time, within the Medical Care Department, working at our City Mission location.
Responsibilities:
Improve patient outcomes and deliver quality care within practice areas as assigned. Provide high quality primary care for predominantly adult patients of Jericho Road Community Health Center Follow up with patients regarding lab results or other medical care concerns Offer patient and family centered care Ensure consistent, cost-effective care Ensure all clinical care is delivered in a manner that meets or exceeds goals and expectations for clinical outcomes, quality assurance standards, state/local regulations, and patient satisfaction. Provide patient care in a wholistic manner including attention to physical, mental, emotional, and spiritual health care Collaborate with interdisciplinary clinical and program teams at Jericho Road Community Health Center Participate in clinical process improvements at our City Mission clinical site related to patient access, population-based quality improvement, and patient experience Create a learning plan that includes familiarity with new research and changes to standard of practice Participate in quality improvement activities with a data-driven approach Attend required staff and provider meetings except when clinical activities prevent such attendance Collaborate with interdisciplinary clinical and program teams at Jericho Road Community Health Center Ensure all actions, personal conduct, and communications represent the organization in a highly professional manner at all times. PSYCHIATRIST
Jericho Road Community Health Center is actively seeking a Psychiatrist. This position is full-time, within the Medical Care Department, working across our Barton, Broadway, & Doat locations.
Responsibilities:
Conduct thorough psychiatric evaluations, including medical history, mental status examinations, and psychosocial assessments to diagnose mental health conditions Develop and implement individualized treatment plans based on patient needs, which may include pharmacotherapy, psychotherapy, and behavioral interventions Prescribe and manage psychotropic medications, including monitoring their effectiveness, potential side effects, and interactions with other medications Provide follow-up care, assess treatment progress, and modify treatment plans as necessary Perform crisis intervention and provide emergency psychiatric care when required Collaborate with patients and their families to educate them about mental health conditions, treatment options, and medication adherence Work with diverse patient populations, including those with co-occurring substance use disorders, trauma, and chronic medical conditions Collaborate with primary care providers, behavioral health team, and other healthcare professionals to deliver integrated, holistic care to patients. Ensure all clinical care is delivered in a manner that meets or exceeds goals and expectations for clinical outcomes, quality assurance standards, state/local regulations, and patient satisfaction. Maintain accurate and timely records of patient care Maintain electronic medical records Keep up to date with professional job knowledge by reading publications, attending educational conferences, or maintaining professional networks.
Qualifications:
Board-certified in psychiatry with license to practice in New York State Experience providing psychiatric treatment to adult and pediatric patients Strong knowledge of evidence-based psychiatric practices Comfortable using telemedicine technology and electronic medical records In-depth knowledge of various psychotherapeutic methods Strong observation skills to accurately evaluate patient’s demeanor and identify symptoms A great bedside manner and the ability to remain calm, empathetic, and patient while interacting with patients who suffer from severe mental disorders
