Overview Provides clinical leadership to promote increasedcompliance with a range of quality and cost measures and standardsof care. Manages service delivery of inter-professional andpara-professional team members working on an individual case orpopulation of cases. Acts as a key resource in providing clinicaland operational guidance and support to assigned teams and otherstaff to achieve and enhance team outcomes. Provides advancednursing clinical care for patients in accordance with current Stateand Federal rules and regulations for nurse practitioner’s scope ofpractice and national standards of care. - Manages and providesfull scope of advanced nursing practice for targeted patientpopulations. Evaluates patient responses to therapy /interventions. Ensures revision of the inter-professional plan ofcare as necessary to achieve quality outcomes.
- Identifies needfor new/revised clinical protocols. Collaborates with physiciansand others within the practice to develop protocols and providestraining as appropriate.
- Manages and provides comprehensiveadvanced nursing care including physical examination, comprehensivehistory, screening for physical and/or psychological conditions,emergent interventions, pharmacological and non-pharmacologicalinterventions, ordering treatments and DME, preventative healthmaintenance activities, care management, referrals, dischargeplanning, counseling and patient education.
Establishes a treatmentplan based on clinical findings and. Determines when furtherevaluation by collaborating physician, specialist or emergency careis warranted.
- Collaborates with patients, families, primary carephysicians and other team members to provide assessment and careplanning. Assesses, plans, and provides intensive and continuouscare management across client settings.
- Manages and providesclinical services in compliance with standards of Patient-CenteredMedical Home standards, meaningful use of medical record data,HEDIS and QARR quality of care measurements.
QualificationsLicenses and Certifications:
- License and current registration topractice as a Registered Professional Nurse in New York Staterequired
- Certificate (license) and current registration topractice as a Nurse Practitioner in the State of New York, with aspecialty in adult health, family health or gerontology required
- Valid driver’s license, as determined by operational/regional needsmay be required
- Maintains credentialed status with VNS HealthMedical Care at Home and associated managed care plans required
- Maintains NPI, Medicaid and Medicare provider numbers preferred
- Maintains a collaborative practice agreement with a physician incompliance with New York State regulations preferred
- Must becertified by ANCC or another accrediting Nurse Practitioner body
- in order to bill Medicare and meet credentialing requirementsrequired
Education:
- Master’s Degree of Science in Nursing, orother graduate degree from a nurse practitioner educational programregistered by the New York State Education Department as qualifyingfor NP certification (licensure) required
- Current ANCC or AANPcertification as an adult, family or geriatric nurse practitionerrequired
Work Experience:
- Minimum of two years of experience as anurse practitioner utilizing full scope of practice preferred
- Clinical home care experience or two years managerial experiencepreferred
- Demonstrated knowledge of Hedis and QARR qualitymeasures, ICD-10 and CPT coding for reimbursement of servicesrequired
- Bilingual skills, as determined by operational needsrequired Pay Range USD $110,000.00
