Family Nurse Practitioner Guardian Health Center Niantic, CT 06357 Overview Guardian Health Center is seeking a compassionate and dedicated Family Nurse Practitioner (FNP) to join our growing team. As a key provider in our patient-centered practice, the FNP will deliver comprehensive care to individuals across the lifespan, with an emphasis on preventive health, chronic disease management, and acute care. Duties Perform physical exams, evaluate patient histories, and order diagnostic tests Develop and implement treatment plans in coordination with supervising physicians Prescribe medications and monitor patient progress Provide health education and counseling for individuals and families Maintain accurate and timely medical documentation Qualifications Licensed as a Family Nurse Practitioner in the state of Connecticut Certification by AANP or ANCC Hospital, Urgent Care, or Emergency Room experience preferred but not required Bilingual (English/Spanish or other) preferred but not required Strong clinical judgment, communication, and teamwork skills
