Family Nurse Practitioner (FNP) – House Call 3.0 3.0 out of 5 stars Brooklyn, NY Job Summary We are seeking a dedicated and compassionate House Call Family Nurse Practitioner (FNP) to join our healthcare team. As a House Call Family Nurse Practitioner, you will provide primary care, wellness visits, chronic disease management, and urgent care services to patients in their homes. focusing on preventive care, health education, and management of chronic conditions. You will work independently with the support of a collaborative healthcare team to deliver high-quality, patient-centered care. This role offers flexibility, autonomy, and the ability to make a positive impact on the lives of patients in their own homes. Duties Must be licensed and credentialed with Medicaid and Medicare in New York Conduct in-home patient assessments and evaluations. Develop and implement individualized care plans. Diagnose and treat acute and chronic medical conditions. Prescribe medications and therapies as needed. Provide health education and counseling to patients and their families. Order and interpret diagnostic tests (labs, imaging, etc.) Collaborate with other healthcare professionals to ensure comprehensive care. Maintain accurate and up-to-date patient records. Provide follow-up care as needed Ensure compliance with all healthcare regulations and standards. Maintain accurate patient records using eCW Adhere to all state and federal healthcare regulations Travel to patient homes in designated service areas Skills Strong knowledge of anatomy and physiology to assess patient conditions effectively. Expertise in managing chronic conditions such as diabetes, hypertension, and heart disease. Ability to provide urgent care for non-emergency situations (e.g., sprains, infections). Skill in diagnosing a wide range of acute and chronic conditions. Proficiency in health coaching techniques to motivate patients towards healthier lifestyle choices. Excellent communication skills to build rapport with patients and families. Ability to work independently as well as part of a collaborative healthcare team. Strong organizational skills to manage multiple patient cases efficiently.
