In this role, you will provide comprehensive mental health care to patients in an ACT Team setting, focusing on behavioral health and memory care. Your expertise will be essential in conducting physical examinations, diagnosing conditions, and developing treatment plans that adhere to established standards of care. Responsibilities Conduct thorough psychiatric evaluations and physical examinations for patients. Develop and implement individualized treatment plans based on patient needs. Provide ongoing patient care, including medication management and therapy. Collaborate with interdisciplinary teams to ensure comprehensive patient care. Educate patients and their families about mental health conditions and treatment options. Monitor patient progress and adjust treatment plans as necessary. Ensure compliance with all regulatory standards and guidelines in the delivery of care. Requirements Master’s degree in Nursing with a specialization in Psychiatric-Mental Health. Current licensure as a Nurse Practitioner in the state of practice. Experience in outpatient behavioral health settings is preferred. Ability to perform physical examinations and assess patient needs effectively. Excellent communication skills, with a focus on patient education and support.
