As an CCM NursePractitioner/ Physician Assistant per diem you will provide care toOptum members and be responsible for the delivery of medical careservices in a periodic or intermittent basis.

PrimaryResponsibilities:
  • Primary Care Delivery
  • Deliver cost-effective,quality care to assigned members
  • Manage both medical andbehavioral, chronic and acute conditions effectively, and incollaboration with a physician or specialty provider
  • Performcomprehensive assessments and document findings in aconcise/comprehensive manner that is compliant with documentationrequirements and Center for Medicare and Medicaid Services (CMS)regulations
  • Responsible for ensuring that all diagnoses areICD10, coded accurately, and documented appropriately to supportthe diagnosis at that visit
  • The APC is responsible for ensuringthat all quality elements are addressed and documented
  • The APCwill do an initial medication review, annual medication review anda post-hospitalization medication reconciliation
  • Facilitateagreement and implementation of the member’s plan of care byengaging the facility staff, families/responsible parties, primaryand specialty care physicians
  • Evaluate the effectiveness,necessity and efficiency of the plan, making revisions as needed
  • Utilizes practice guidelines and protocols established by CCM
  • Must attend and complete all mandatory educational and LearnSourcetraining requirements
  • Travel between care sites mandatory
  • CareCoordination
  • Understand the Payer/Plan benefits, CCM associatepolicies, procedures and articulate them effectively to providers,members and key decision-makers
  • Assess the medicalnecessity/effectiveness of ancillary services to determine theappropriate initiation of benefit events and communicate theprocess to providers and appropriate team members
  • Coordinate careas members transition through different levels of care and caresettings
  • Monitor the needs of members and families whilefacilitating any adjustments to the plan of care as situations andconditions change
  • Review orders and interventions forappropriateness and response to treatment to identify mosteffective plan of care that aligns with the member’s needs andwishes
  • Evaluate plan of care for cost effectiveness while meetingthe needs of members, families and providers to decreases highcosts, poor outcomes and unnecessary hospitalizations
  • ProgramEnhancement Expected Behaviors
  • Regular and effectivecommunication with internal and external parties includingphysicians, members, key decision-makers, nursing facilities, CCMstaff and other provider groups
  • Actively promote the CCM programin assigned facilities by partnering with key stakeholders (i e :internal sales function, provider relations, facility leader) tomaintain and develop membership growth
  • Exhibit original thinkingand creativity in the development of new and improved methods andapproaches to concerns/issues
  • Function independently andresponsibly with minimal need for supervision
  • Ability to enteravailable hours into web-based application, at least one monthprior to available work time
  • Demonstrate initiative in achievingindividual, team, and organizational goals and objectives
  • Participate in CCM quality initiatives

Required Qualifications:
  • Certified Nurse Practitioner orPhysician Assistant through a national board
  • For NPs:

    Graduate ofan accredited master’s degree in Nursing (MSN) program and boardcertified through the American Academy of Nurse Practitioners(AANP) or the American Nurses Credentialing Center (ANCC),Adult-Gerontology Acute Care Nurse Practitioners (AG AC NP),Adult/Family or Gerontology Nurse Practitioners (ACNP), withpreferred certification as ANP, FNP, or GNP

  • For PAs:

    Graduate ofan accredited Physician Assistant degree program and currentlyboard certified by the National Commission on Certification ofPhysician Assistants (NCCPA)

  • Active and unrestricted license inthe state which you reside
  • Current active DEAlicensure/prescriptive authority or ability to obtain post-hire,per state regulations (unless prohibited in state of practice)
  • Access to reliable transportation that will enable you to travel toclient and/or patient sites within a designated area
  • Ability tolift a 30-pound bag in and out of car and to navigate stairs and avariety of dwelling conditions and configurations
  • Availability towork 24 hours per month, with expectations that 16 of the 24hours/month could be during off-hours (after 5 pm, on weekends,and/or holidays) not to exceed 960 hours in a calendar year
  • Ability to gain a collaborative practice agreement, if applicablein your state
Preferred Qualifications:
  • 1 years of hands-on postgrad experience within Long Term Care
  • Understanding of Geriatricsand Chronic Illness
  • Understanding of Advanced Illness and end oflife discussions
  • Proficient computer skills including the abilityto document medical information with written and electronic medicalrecords
  • Ability to develop and maintain positive customerrelationships
  • Adaptability to change The hourly range for thisrole is $43.

17 to $84.95 per hour based on full-time employment.

Pay is based on several factors including but not limited to locallabor markets, education, work experience, certifications, etc.
