TheSenior Community Care (SCC) program is a longitudinal, integratedcare delivery program that coordinates the delivery and provisionof clinical care of members in a facility setting. The APC withinSCC provides care to our highest-risk health plan members and ispart of an interdisciplinary team. In this role you will have theability to achieve work life balance. No on-call, no weekends, andno holidays required. This position requires travel through thestate of New York.
Primary Responsibilities:
- Provide clinicalcoverage for the primary APC during periods of PTO or short/longterm absences
- Manage members through an acute medical situationor an exacerbation of a chronic condition, including testing,diagnostics, and interventions
- Manage members during a shortterm, sub-acute rehabilitation episode
- Conduct comprehensiveassessments
- Effectively manage acute and chronic medical andbehavioral conditions
- Ensure accurate and complete documentation,including ICD 10 conditions
- Communicate and collaborate with theinterdisciplinary care team
- Conduct advanced illness and advancedcare planning conversations

Required Qualifications:
- Active and unrestricted license in thestate of Florida or ability to obtain by start date
- CertifiedNurse Practitioner or Physician Assistant through a national board

For NPs:
Graduate of an accredited master’s degree in Nursing(MSN) program and board certified through the American Academy ofNurse Practitioners (AANP) or the American Nurses CredentialingCenter (ANCC), Adult-Gerontology Acute Care Nurse Practitioners (AGAC NP), Adult/Family or Gerontology Nurse Practitioners (ACNP),with preferred certification as ANP, FNP, or GNP

For PAs:
Graduate of an accredited Physician Assistant degree program andcurrently board certified by the National Commission onCertification of Physician Assistants (NCCPA)
- Current, active DEAlicensure/prescriptive authority or ability to obtain post-hire,per state regulations (unless prohibited in state of practice)
- 1years of hands-on post grad experience (long-term care settingpreferred)
- Ability to gain a collaborative practice agreement, ifapplicable in your state
- Ability to complete physicalrequirements of the job including lifting a 30-pound bag in and outof the car, navigating stairs and a variety of dwelling conditions,and sit, stand, and kneel to perform physical assessment
- Driver’sLicense and access to reliable transportation
PreferredQualifications:
- Experience in meeting the medical needs ofpatients with complex behavioral, social, and/or functional skills
- Understanding of Geriatrics and Chronic Illness
- Understandingof Advanced Illness and end of life discussions
- Proficientcomputer skills including the ability to document medicalinformation with written and electronic medical records The salaryrange for this role is $89,800 to $176,700 annually based onfull-time employment.
Pay is based on several factors including butnot limited to local labor markets, education, work experience,certifications, etc. UnitedHealth Group complies with all minimumwage laws as applicable. Employees must be in an active regularstatus. If an employee leaves Home and Community, the student loanrepayments will cease. The employee must remain in an AdvancedPractice Clinician or Physician role within Home and Community for36 months to receive the full benefit of the student loanrepayments.
