Family Practice/Primary Care Nurse Practitioner Position Available In Nassau, New York

• Board Certified Nurse Practitioner with experience
• Bi-lingual English / Spanish required
• Provide effective and equitable value-based primary care to adults on Medicare to keep them happy, healthy, and out of the hospital.
• Nurse Practitioners will see patients independently and collaborate with physicians, depending on the patients specific situation.
• Outpatient Setting
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments, which require aholistic view of health and afocus on thoughtful, accurate, and specific documentation.
• Conducting home visits as needed.
• Experience in primary care: internal medicine, geriatrics, or family medicine
• Understands HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) Coding, and Health Risk Assessments (HRAs).
