Overview Provides clinical leadership to promote increasedcompliance with a range of quality and cost measures and standardsof care. Manages service delivery of inter-professional andpara-professional team members working on an individual case orpopulation of cases. Acts as a key resource in providing clinicaland operational guidance and support to assigned teams and otherstaff to achieve and enhance team outcomes. Provides advancednursing clinical care for patients in accordance with current Stateand Federal rules and regulations for nurse practitioner’s scope ofpractice and national standards of care.   • Manages and provides the fullscope of advanced nursing practice for targeted patientpopulations, which may include short-term treatment of members atthe point of care. Evaluates patient responses totherapy/interventions. Ensures revision of the inter-professionalplan of care as necessary to achieve quality outcomes.
  • Serves asthe clinical authority on individualized care teams and leadsrounds and interdisciplinary care team meetings.
  • Manages andprovides comprehensive, advanced nursing care, includingpost-discharge aftercare visits, annual comprehensive assessmentvisits, palliative care-care management program onboarding, andchange in condition assessment visits (including physicalexamination, comprehensive history, screening for physical and/orpsychological conditions, and point of care testing).

Urgentinterventions (i.e., escalations for the Care Teams, RPM, and the24/7 Line), pharmacological and non-pharmacological interventions,ordering treatments and DME, preventative health maintenanceactivities, care management, referrals, discharge planning,counseling, and patient education. Establishes a treatment planbased on clinical findings and determines when further evaluationby the collaborating physician, specialist, or emergency care iswarranted.

  • Collaborates with patients, families, primary carephysicians and other team members to provide assessment and careplanning. Assesses, plans, and provides intensive and continuouscare management across client settings.
  • Manages and providesclinical services in compliance with standards of Patient-CenteredMedical Home standards, meaningful use of medical record data,HEDIS and QARR quality of care measurements.
  • Managesinter-professional team efforts regarding the medical, nursing,therapy and ancillary care provided to patients to ensure qualityoutcomes are achieved.
  • Participates in on call coverage scheduleto ensure 24/7 access to practice clinicians.
  • Assesses, educates,and improves client and caregiver knowledge of chronic disease,self-care management, and identification of changes in healthstatus, including appropriate responses and actions throughindividualized education and inter-professional interventions.
  • Observes and analyzes team performance patterns related topopulation under care and assists in developing interventions toimprove team performance. Provides remedial support and guidance tointerdisciplinary team members to address implementation/evaluationof plans that maintain/increase customer experience with care,cost-efficiency, and quality care compliance with regulatorystandards.
  • Communicates with internal and external care partnersregarding the needs of the patient or population to ensureinterventions occur in a timely and appropriate manner. Intervenesas needed when the care plan is not executed and remediates thesituation to prevent reoccurrence.
  • Performs procedures asoutlined in collaborative practice agreement and as privileged bythe Credentialing Committee.
  • Manages quality of medical recorddocumentation and submits billing information in accordance withProfessional Corporation policy.
  • Provides care in one or morecare settings based on the clinical requirements: virtually,telephonically or travels to patients’ homes and/or otherfacilities with varying environments (e.g., elevated buildings,walk-ups, care facilities, single/multiple family homes, presenceof pets, etc.) using approved transportation options.
  • Uponrequest, performs peer reviews of work performance, includingquality of care, clinical documentation, coding and billingpractices, communication skills, and population surveillance.
  • Mayparticipate in the VNS Health Medical Care at Home Quality andProfessional Advisory Committee and any associated workgroupsrelated to development of evidence based clinical models of care,peer education and training, quality improvement, medical recordconfiguration and reporting, maximizing the use of clinicaldecision support systems.
  • May act as a preceptor for studentNurse Practitioners, providing excellent role modeling ofcommunity-based primary care practice.
  • Assumes responsibility forcontinued professional growth, and maintains professionalcertification and licensure.
  • For Psychiatric Nurse Practitionersonly:
  • Conducts psychosocial assessments of interventions patientsand develops treatment recommendations. Determines whether furtherevaluation by a collaborating psychiatrist is needed.
  • Providesthe following services to patients: advocacy, psychoeducation,psychopharmacology, cognitive/behavioral interventions, monitoringof mental status, emotional support, brief psychotherapy andfamily-based interventions.
  • Supports and educates VNS Healthstaff and caregivers to enable them to better care for patientswith psychiatric, cognitive, and/or behavioral challenges.
  • Consults with a collaborating psychiatrist as necessary to developpsychiatric evaluation and treatment plans.
  • Consults with thereferrer on assessment, recommendations, and updates on treatmentprogress.
  • Follows treatment plans, revising and updating them asnecessary, makes appropriate referrals for additional mental healthservices and provides crisis intervention as required.
  • Participates in interdisciplinary team meetings, case conferencesand clinical supervision; attends in-service education programs.
  • Participates in special projects and performs other duties asassigned.
Qualifications Additional Job Description Licenses andCertifications:
  • License and current registration to practice as aRegistered Professional Nurse in New York State required
  • Certificate (license) and current registration to practice as aNurse Practitioner in the State of New York, with a specialty inadult health, family health or gerontology required
  • Validdriver’s license, as determined by operational/regional needs maybe required
  • Maintains credentialed status with VNS Health MedicalCare at Home and associated managed care plans required
  • MaintainsNPI, Medicaid and Medicare provider numbers preferred
  • Maintains acollaborative practice agreement with a physician in compliancewith New York State regulations preferred
  • Must be certified byANCC or another accrediting Nurse Practitioner body
  • in order tobill Medicare and meet credentialing requirements required
  • ForPsychiatric Nurse Practitioners only:
  • Current PMHNP-BCcertification required
Education:
  • Master’s Degree of Science inNursing, or other graduate degree from a nurse practitionereducational program registered by the New York State EducationDepartment as qualifying for NP certification (licensure) required
  • Current ANCC or AANP certification as an adult, family orgeriatric nurse practitioner required
  • For Psychiatric NursePractitioners only:
  • Master’s Degree in psychiatric-mental healthNurse Practitioner required
  • PhD in psychiatric-mental healthNurse Practitioner preferred
Work Experience:
  • Minimum of twoyears of experience as a nurse practitioner utilizing full scope ofpractice preferred
  • Clinical home care experience or two yearsmanagerial experience preferred
  • Demonstrated knowledge of Hedisand QARR quality measures, ICD-10 and CPT coding for reimbursementof services required
  • Bilingual skills, as determined byoperational needs required

  • Minimum of two years of experience as a psychiatric nursepractitioner utilizing full scope of practice required Pay RangeUSD $110,000.00
