PrimaryResponsibilities:
  • Conduct in-home assessments including anevaluation of past medical history, review of systems, medicationreconciliation, vital signs, and comprehensive physical exam
  • Perform evidence-based practice screenings including point of caretesting (as appropriate)
  • Identify diagnoses to be used in caremanagement and active medical management in the furtherance oftreatment
  • Formulate a list of current and past medical conditionsusing clinical knowledge and judgment and the findings of yourassessment
  • Communicate findings in your assessment that will beused to inform member’s PCP of potential gaps in care
  • Identifyurgent and emergent situations and intervene appropriately
  • Educate members on topics such as disease process, medication, andcompliance You’ll be rewarded and recognized for your performancein an environment that will challenge you and give you cleardirection on what it takes to succeed in your role as well asprovide development for other roles you may be interested in.
Required Qualifications:
  • Active NP or PA license or ability toobtain by start date. Licensure must be unencumbered, free of anyopen/unresolved disciplinary actions including probation orrestrictions against privilege to practice
  • Active ANCC or AANPnational certification in Family, Geriatrics, Adult,Adult-Gerontology Primary Care, or Adult-Gerontology Acute Carecertification, or the ability to obtain national certificationand/or NP or PA license in state of assignment by start date
  • Ability to complete physical requirements of the job includingmoving a 30-pound bag in and out of the car, navigating stairs anda variety of dwelling conditions, remain in a stationary positionand position self to perform physical assessment
  • Driver’s licenseand access to a reliable transportation to complete home visitassessments
Preferred Qualifications:
  • May be requested to obtainadditional licensure in other geographic areas
  • 1 years ofclinical experience in their highest level of education, clinicalsetting
  • Experience in gerontology, cardiology, internal medicine,or endocrinology
  • Home Health care or home visit experience
  • Ability to travel up to 5% within licensed states if requested foradditional incentives (with notice, based on business needs)
PLEASENOTE

Employees must be in an active regular status. Employees mustremain in role for a minimum of 12 months from the date of hire/rehire/transfer. If an employee leaves Home and Community, thestudent loan repayments will cease. The employee must remain in anAdvanced Practice Clinician or Physician role within Home andCommunity for 36 months to receive the full benefit of the studentloan repayments. The salary range for this role is $104,500 to$156,000 annually based on full-time employment. Salary Range isdefined as total cash compensation at target. The actual range andpay mix of base and bonus is variable based upon experience andmetric achievement. Pay is based on several factors including butnot limited to local labor markets, education, work experience,certifications, etc. UnitedHealth Group complies with all minimumwage laws as applicable.
