PrimaryResponsibilities:

• Conduct in-home assessments including anevaluation of past medical history, review of systems, medicationreconciliation, vital signs, and comprehensive physical exam
• Perform evidence-based practice screenings including point of caretesting (as appropriate)
• Identify diagnoses to be used in caremanagement and active medical management in the furtherance oftreatment
• Formulate a list of current and past medical conditionsusing clinical knowledge and judgment and the findings of yourassessment
• Communicate findings in your assessment that will beused to inform member’s PCP of potential gaps in care
• Identifyurgent and emergent situations and intervene appropriately
• Educate members on topics such as disease process, medication, andcompliance

Required Qualifications:

• Active NP or PA license or abilityto obtain by start date. Licensure must be unencumbered, free ofany open/unresolved disciplinary actions including probation orrestrictions against privilege to practice
• Active ANCC or AANPnational certification in Family, Geriatrics, Adult,Adult-Gerontology Primary Care, or Adult-Gerontology Acute Carecertification, or the ability to obtain national certificationand/or NP or PA license in state of assignment by start date; ORfor PAs
• Physician Assistant national certification through NCCPA
• Ability to complete physical requirements of the job includinglifting a 30-pound bag in and out of the car, navigating stairs anda variety of dwelling conditions, and sit, stand and kneel toperform physical assessment
• Reliable transportation to completehome visit assessments

Preferred Qualifications:

• 1 year ofclinical experience in their highest level of education, clinicalsetting
• Experience in gerontology, cardiology, internal medicine,or endocrinology
• Home Health care or home visit experience
• Ability to travel up to 5% within licensed states if requested foradditional incentives (with notice, based on business needs)
• Maybe requested to obtain additional licensure in other geographicareas The salary range for this role is $104,500 to $156,000annually based on full-time employment.

Salary Range is defined astotal cash compensation at target. The actual range and pay mix ofbase and bonus is variable based upon experience and metricachievement.
