Immediate Hire (Virtual Interviews Available) Why This Role Matters Were hiring a Nurse Practitioner to serve patients in long-term care (LTC) and skilled nursing facilities (SNF) throughout Queens . This role supports older adults with complex medical needs, providing proactive care that helps reduce hospitalizations and improves overall outcomes. You will make a direct impact by: Identifying and closing clinical gaps Leading care coordination in collaboration with facility staff Educating patients and families on conditions and plans Supporting continuity of care between facilities and home Key Responsibilities Perform comprehensive one-time assessments Conduct physical exams, medication reviews, and cognitive screenings Develop individualized treatment plans and identify risks Lead goals-of-care conversations (training available) Communicate with PCPs, specialists, and facility care teams Facilitate safe transitions across levels of care
Ideal Candidate Profile Licensure:
Active New York NP license
Experience:
Minimum of 2 years in geriatrics, SNF, LTC, or internal medicine
Skills:
Excellent assessment and documentation skills Strong collaboration and communication abilities Comfort with facility-based rounding and community travel
