Job Description:

Primary Vascular Care is looking for a Nurse Practitioner (NP)or Physician Assistant (PA) to perform on-site clinical assessmentsto determine appropriate patient treatment plans of residents ofclient facilities. This position reports to the Lead NursePractitioner. The NP/PA will work in a mobile practice, primarilyserving a long-term care setting. This position is 30-40 hours aweek

ESSENTIAL FUNCTIONS

Performs on-site screening clinicalassessments to determine appropriate patient treatment plans ofresidents of client facilities by the completion of a comprehensiveassessment form Functions independently to perform appropriatehistory and physical for complex acute, critical, and chronicallyill perioperative patients Orders and reviews diagnostic andtherapeutic tests relative to patient’s age-specific needs Assistswith the provision of care in accordance with facility, state, andfederal regulations Monitors the effectiveness of interventionsMaintains relationships with all appropriate contacts in facilitiesEducates family members, clients, and prospective accountsregarding services and programs offered by Primary Vascular CareAssists with the development and maintenance of physicianrelations, as assigned Completes weekly, monthly, and otherreports, as assigned, in a timely and comprehensive manner

QUALIFICATIONS

Bachelor’s Degree required Degree from anaccredited Nurse Practitioner or Physician Assistant Programrequired Current NP License required BLS – Basic Life Supportthrough American Heart Association required ACLS – Advanced CardiacLife Support through American Heart Association preferred CurrentInfection Control Certification required Board Certified/BoardEligible required

Physical, Vaccines and Titers:

PPD/TB, Hep B,MMR, Varicella required COVID and Flu Vaccine preferred 1 year ofNP experience preferred
