This role is ideal for Nurse Practitioners passionate about improving outcomes for older adults by: Identifying gaps in care Educating patients and families Coordinating comprehensive care plans Key Responsibilities Perform one-time, in-depth assessments (physical exams, med rec, ADLs, cognitive/nutritional evaluations) Develop evidence-based treatment plans and problem lists Facilitate goals-of-care conversations (training provided) Collaborate with PCPs, facility teams, and families Support safe transitions of care to home or other facilities
Ideal Candidate Profile Licensure:
Active Pennsylvania NP license
Experience:
2+ years in geriatrics, primary care,
LTC, or SNF Skills:
Advanced clinical assessment capabilities Excellent communication across diverse teams and families Comfortable with local travel between multiple facilities
