As a Nurse Practitioner the candidate will conduct home visits to provide primary care assessments, follow-ups, and preventive care for elderly patients. This is a field-based role, not clinic-based, and requires travel to patient homes throughout the region.

Responsibilities:

Conduct primary care visits in patients’ homes Perform routine follow-ups, wellness checks, and preventive care assessments Document patient visits accurately and promptly Collaborate with other providers to ensure comprehensive, coordinated care Maintain regular communication with Medical leadership and support staff

Qualifications:

Licensed Nurse Practitioner in the state of Pennsylvania (required) Minimum 9 months-1 year of experience in geriatric or primary care Must have reliable transportation and valid driver’s license Strong interpersonal and clinical assessment skills Comfortable working independently in patients’ homes

Schedule:

Full-time:

Monday-Friday, 9:00 AM – 5:00 PM Sunday availability is a major plus
