Charlotte Hungerford Hospital is a 122-bed, general acute carecommunity hospital located in Torrington, Connecticut, that servesas a regional health care resource for the 100,000 residents ofLitchfield County and Northwest Connecticut. POSITION SUMMARY/PURPOSE
The APRN is responsible forproviding primary care services, including assessing, diagnosing,prescribing, treating, and educating patients. Works closely withphysicians and other clinicians in a team approach to patient care.
May share in the supervision of RNs, LPNs, and/or medicalassistants, depending on departmental organization.
ESSENTIALFUNCTIONS 1.
Assesses patient status by obtaining health historythrough patient/family interviews and chart reviews; assessingpresenting illness, risk factors, and family history, psychosocialsituation, and cultural factors; and performing appropriatephysical examination. Orders/performs appropriate laboratorydiagnostic and other screening tests. Seeks other information asneeded, including consultation with physicians and otherclinicians, for evaluation of illness. Integrates data to determinediagnosis and therapeutic plan, including identification of anyhealth risks. Develops and implements treatment plan byprescribing/dispensing medications and/or injections in compliancewith medical practice guidelines and state laws. Handles minorlacerations. Instructs patient/family regarding medications andtreatments. Educates patients regarding health promotion/illnessprevention. Recommends appropriate community resources to meetpatient/family needs. Communicates appropriate case managementinformation to other professionals and community agencies. Preparesdocumentation for medical records including updating patientmedical chart by posting examination and test results, diagnosis,medications, and treatment in written/computerized manner.
Participates in peer reviews, chart reviews, staff education,clinical guideline development, and other continuing education andquality assurance activities to demonstrate compliance withstandards, regulations, policies, and procedures. Acts asconsultant/collaborator to clinical team on area of specialty.
Complies with patient confidentiality requirements. Promotespatient advocacy. Collaborates with physicians in managing acuteand long-term medical needs of patient. Provides monitoring andcontinuity of care between visits according to treatment planincluding triaging patient calls/e-mails.
JOB KNOWLEDGE, SKILLREQUIREMENTS, AND DESIRED ATTRIBUTES
Knowledge:
Knowledge ofadvanced practice of nursing including anatomy and physiology,pathophysiology, pharmacology and therapeutics, basic nutrition,behavioral factors, psychosocial/family systems, diagnostictesting, interpretation of results and clinical decision making,alternative treatment modalities. Knowledge of professional nursingtheory, practices and regulations related to evaluating andproviding patient care, and professional ethics related to thedelivery of nursing care. Knowledge of medical equipment andinstruments used to administer patient care. Knowledge of commonsafety hazards and precautions to establish/maintain a safe workenvironment. Knowledge of health promotion, health riskidentification, and patient education principles and techniques.
Knowledge of community, professional, and educational resources.
Skills:
Skill in oral and written communication includinginterviewing to gather medical histories; providing presentations,consultations, facilitation, and documentation, e.g., writingmedical orders, patient education materials, and medical recordupdates. May require skill in using computerized health informationmanagement systems. Skill in case management, time management,problem solving, crisis intervention, multitasking, and otherorganizational matters. Skill in modifying methods and techniquesof professional nursing to provide appropriate nursing care and ininitiating appropriate emergency response and investigatoryprocedures.
Abilities:
Ability to analyze complex data and drawconclusions. Ability to interpret, adapt, and apply guidelines andprocedures. Ability to establish/maintain effective workingrelationships with patients, clinical staff, and the public.
Qualifications
EDUCATION BSN
requires; MSN preferred. Successfulcompletion of advanced practitioner training and certification as apractitioner (general or specialized) from an approved nursepractitioner program. Current state license as an advancedregistered nurse practitioner with prescriptive authority. CurrentCPR certification.
EXPERIENCE
Minimum three years of professionalnursing experience in a clinic setting, plus two years of nursepractitioner experience. Current state registered nurse license. Wetake great care of careers.
