The Home and Community Care (HCC) program is a longitudinal,integrated care delivery program. The HCC National On Call advancedpractice clinician (APC) is responsible for providing telephoniccare and direction to members and facility staff during variousevening, overnight, weekend, and holiday hours. This role isresponsible for the delivery of medical care services to apre-designated group of enrollees. In this remote role you willprovide afterhours virtual (primarily telephonic) care for agingresidents in various settings. Scheduling:
  • This is a full time, work from homeposition requiring variable shift coverage on weeknights, weekends,and holidays
  • While your shift times can vary, we provide coverageto members:
  • on weeknights from 5pm
  • 8am local time, and
  • continuous weekend coverage from Friday at 5pm to Monday at 8am,and
  • Holiday coverage beginning at 5pm of the end of the lastbusiness day to 8am of the resumption of business hours
  • WeekdayShifts are assigned between 5PM and 8AM
  • Weekend Shifts areassigned between 12AM and 12PM
  • Holiday scheduling is completed atthe beginning of the year for advanced planning
ExperiencePreference:

Candidates with recent experience in high-acuitygeriatric care (e.g., SNF, LTACH, Hospitalist, ICU, Emergency) arepreferred.

You will receive about 4 after-hourscalls per hour on average from

Nursing Home Staff, RNs, DONs, StaffTyping Skills:

Excellent typing skills are essential as you willneed to enter notes live for each case If you are located in NY orNJ License, willing and able to work after normal business hoursincluding weekends and holidays, you will have the flexibility towork remotely

  • as you take on some tough challenges.
PrimaryResponsibilities:
  • Available on provided telephonic platform, bothtaking and placing calls to coordinate care for members betweenfacilities, hospitals, and Optum field colleagues
  • Utilize EMRproficiently to provide acute care to members during overnight andweekend and holiday hours
  • Primary Care Delivery
  • Manage bothmedical and behavioral, chronic, and acute conditions effectively,and in collaboration with a physician or specialty provider
  • Responsible for ensuring encounter is documented appropriately tosupport the diagnosis at that visit
  • Attend and complete allmandatory educational training requirements
  • Care Coordination
  • Monitor the needs of members and families while facilitating anyadjustments to the plan of care as situations and conditions change
  • Review orders and interventions for appropriateness and responseto treatment to identify the most effective plan of care thataligns with the member’s needs and wishes
  • Program EnhancementExpected Behaviors
  • Regular and effective communication withinternal and external parties including physicians, members, keydecision-makers, nursing facilities, HCC staff and other providergroups

Required Qualifications:
  • Active NY or NJ NP license or ability toobtain by start date. Licensure must be unencumbered, free of anyopen/unresolved disciplinary actions including probation orrestrictions against privilege to practice, or ability to obtain bystart date
  • Active ANCC or AANP national certification in Family,Geriatrics, Adult, Adult-Gerontology Primary Care, orAdult-Gerontology Acute Care certification, or the ability toobtain national certification and/or NP license in state ofassignment by start date
  • Current active DEA licensure or abilityto obtain post-hire, per state regulations (unless prohibited instate of practice)
  • Availability to work assigned weekends,weekdays, and pre-assigned holidays
Preferred Qualifications:
  • 2years of hands-on postgrad experience within Long Term Care
  • Understanding of Geriatrics and Chronic Illness
  • Understanding ofAdvanced Illness and end of life discussions
  • Proficient computerskills, including the ability to document medical information withwritten and electronic medical records
  • All employees workingremotely will be required to adhere to UnitedHealth Group’sTelecommuter Policy The salary range for this role is $104,500 to$156,000 annually based on full-time employment.

Salary Range isdefined as total cash compensation at target. The actual range andpay mix of base and bonus is variable based upon experience andmetric achievement. Pay is based on several factors including butnot limited to local labor markets, education, work experience,certifications, etc. UnitedHealth Group complies with all minimumwage laws as applicable.
