FHCN seeks a Nurse Practitioner to provide primaryhealth care to individuals and families at the Practitionersassigned site and other satellite sites as assigned. This positionwill require the ability to travel with our Outreach department tovarious communities within the tri-county area.

ESSENTIAL DUTIES ANDRESPONSIBILITIES

(Included but not limited to thefollowing);Provides direct, high-quality, primary medical care toFHCN patients; maintains and enhances a current level of knowledgerelative to professional practice; and continually strives toimprove the quality of care at FHCN.Provide or arrange forcomprehensive primary care services in a primary medical careambulatory clinic setting, including laboratory and diagnosticworkup, and specialty referrals.

Uses his or her best professionaljudgment while practicing medicine at FHCN within his or her scopeof practice. Works interdependently with other health careproviders and health care staff in a team environment.

Perform minoroutpatient procedures in accordance with FHCNcredentialing/privileging policy.

Provide consultation to otherclinic providers either in person or by telephone.

Consult with andcoordinate patient care with clinic physicians, covering communityprimary care physicians, specialists, pharmacists, nurses, or otherhealth care professionals as appropriate.

Assess patient needs andarrange for specialty referrals, diagnostic and in-patient care andmake appropriate referrals for service.

Responsible for accurate,timely, and complete documentation in patient records in accordancewith FHCN policy.

Provide patients and family members withcounseling and education regarding health maintenance, diseaseprevention, diagnosis, treatment, and need for follow up asappropriate during each patient visit. Verify and document that thepatient understands diagnosis, treatment and follow uprecommendations.

Renders care to FHCN patients in accordance withquality and standards as are established by FHCN, the standards ofcare prevailing in the community, applicable law, and according tostate and federal grant requirements for non-discriminationregardless of status, race, sex, religion, national origin,handicap, age, or ability to pay.

Collaborates with other members ofthe multidisciplinary health care team to identify and address thecomplex issues which may adversely impact the health of homelessclients.

Participates as an effective team member in the provisionof medical and health care.

Facilitates client participation inself-care by providing information and choices in a non-judgmental,respectful manner.

Takes initiative when appropriate to determineways to deliver health care in a more compassionate, expedited,user-friendly manner.

Maximizes community linkages while acting asadvocate for patients.

Provides after-hours care via call serviceand during extended hours and weekends as scheduled.

KNOWLEDGE,SKILLS AND ABILITIES REQUIRED

Ability to function effectively as ateam member and facilitate completion of tasks.

Experience in FQHC(Federally Qualified Health Center) industry preferredRequiresstrong organizational skills, problem solving and analyticalskills.

Must be computer literate in accounting software excel, wordand outlook.

Able to exercise sound judgment and discretion inmanaging patient and employee issues.

Requirements:
EDUCATION

ANDEXPERIENCEMinimum of 2 years private practice experience or 1 yearexperience in a community health center.

Spanish bilingualpreferred.

Current registration in the State of South Carolina as anAdvanced Practice Nurse.

Certification as a Nurse Practitioner fromthe American Nurses Credentialing Center.

A Master of Science degreein Nursing from a program accredited by the American NursesAssociation of the American Association of Nurse Practitioners.
