About the

Position:

During a visit that can last up to onehour, Matrix providers review and observe a members current health,medical history, medication adherence, social environment and otherrisks. This provides unmatched insight into a members overallhealth and well-being that can be difficult to capture duringroutine office visits. The Matrix Comprehensive Health Assessmenthelps to improve quality of care and allows us to potentially closemultiple care gaps with a single visit.

Expect:

Conduct Adult/Geriatric assessments toinclude medical history, diagnosis and treatment, health education,physician referrals, case management referrals, follow-up and cleardocumentation according to Matrix guidelines and protocolsWorkcollaboratively with physicians, case managers, social workers,family members, key caregivers, and ancillary medical personnel asappropriateCollaborate with Primary Care Physician (PCP) on patienteducation, provide follow-upProvide services in a variety of venuesto include: Home Visits, Skilled Facility VisitsNurse Practitioner

Required:

Masters DegreeOR commensurate experience and satisfactory completion of NPlicensureCurrent RN and NP licensure in state of practice toinclude prescription authority or the ability to obtainprescriptive authorityBoard certified by the AANP, ANCC or theAACCN in a Matrix approved specialtyCurrent BLS, ACLS or CPRcertificationMinimum of 1 year experience as a NursePractitionerStrong computer skills and familiarity with employeehealth/medical record softwareExcellent verbal and writtencommunication skills with patient, clients, and colleaguesComfortand flexibility with frequent change
