Job Description

Job Description McLeod Hospice and Palliative Medicine is seeking an Outpatient Palliative Nurse Practitioner! •Minimum of 2 years experience as a Nurse Practitioner is required to be considered for the position•

Qualifications/Training:

Possession of valid South Carolina driver’s license and an available vehicle at all times. Must have 2 years of Nurse Practitioner experience.

Licenses/Certifications/Registrations/Education:

South Carolina Nurse Practitioner certification. Master’s degree.

Essential Job Responsibilities/Expectations:

1. Job responsibilities include those listed in competency document. 2. Maintains a professional image and exhibits excellent customer relations to patients, visitors, physicians, and co- workers in accordance with our Service Excellence Standards and Core Values. 3. Provides Palliative Care to patients in collaboration with the Palliative Medicine Physicians and by following established medical protocols. 4. Plans, implements, and coordinates the care of Palliative care patients in the outpatient setting. 5. Provides direct NP services, in a direct face to face visit, or via telehealth with patients and families. 6. Performs physical examinations and treatments within prescribed guidelines and instructions of the physicians. 7. Reduces readmission rates, monitors medication effectiveness, assists with transitions of care, improves the quality of patient care. 8. Coordinates patient assessment and subsequent visits. 9. Provides teaching and counseling concerning patients health, disease progression, symptom management and the patients response to care. 10. Orders and interprets diagnostic tests to identify and assess patients clinical problems. 11. Records physical findings and formulates plans and prognosis, based on patient’s condition.
