Job Description
PRN NP – Home Health Visits, SC 3.1 3.1 out of 5 stars Orangeburg, SC 29118 South Carolina we’re hiring passionate and self-driven Nurse Practitioners to perform Comprehensive Health Assessments (CHAs) for In-Home and Facility-Based Patients!
Requirements:
Board certification (ANCC or AANP) as an
FNP, AGNP
Active SC license Current BLS certification Strong clinical judgment and ability to work independently in the field Comfortable with electronic documentation and tablet use Willingness to travel extensively within the assigned area (overnight travel may be required) New graduates welcome to apply!
Job Details:
Perform in-home or facility-based CHAs for Medicare, Medicaid, and commercial members Conduct health screenings such as A1C, KED, MAU, FIT, PAD, and DRS, as appropriate Identify and escalate care gaps and urgent/emergent health concerns Provide preventive health education and coaching Complete thorough electronic documentation during the visit Communicate regularly with leadership and stay current with updates Travel within a 60-mile radius from your assigned location (daily mileage may exceed 120 miles) Use a provided tablet for assessments and communication
Work Environment:
Visits occur in homes, skilled nursing, and assisted living facilities Must be comfortable working with diverse populations and home environments Safety-first approach: GPS-enabled safety app provided Reliable transportation required — work may involve all weather conditions and varied terrain What Our client has to offer:
PRN Role:
