40/week We are seeking a Nurse Practitioner to serve as an inpatient Palliative Care consultant within local area hospitals to provide excellent patient care through optimized symptom management and complex discussions surrounding goals of care or advance care planning. Hospice or palliative care experience is strongly suggested, as well as a desire to independently build a strong referral base within the assigned facility. SUMMARY The Advanced Practice Provider (APP) works independently, and in collaboration with, the Alive supervising physician to deliver excellent palliative care alongside the care of the hospital-based medical team. The APP provides palliative medical services by managing the symptoms related to acute and chronic conditions, assisting with communication and coordination of care, as well as developing treatment plans. This includes collaboration with the medical team, social workers, case managers, chaplains, etc. to optimize holistic care of the patient. The APP would provide recommendations for and/or orders including pharmaceutical interventions/medications, laboratory, and diagnostic services as needed, and will work closely with the patient and family to provide disease specific education and to establish goals of care. Due to a referral-based census, the APP will exhibit excellent customer service and professionalism to all patients and referral sources, as well as ambition to build and grow referral source relationships. The APP must have a positive attitude, display comfort in autonomy, demonstrate initiative, flexibility, and be team oriented.
ESSENTIAL DUTIES AND RESPONSIBILITIES 1.
Under the direction of the designated supervising physician, diagnoses medical conditions, plans and implements interventions, and evaluates patient response. 2. Assembles and documents complete patient medical database including health history and physical examination. 3. Documents assessments, orders, interventions, results and discharge planning in the medical record. 4. Communicates findings with consulting physicians and IDT (interdisciplinary team) members as appropriate. 5. Provides information, education, and support to patients and families. 6. Facilitates communication between patients, family members, and the IDT. 7. Fosters teamwork and serves as a resource within the IDT. 8. Promotes and practices economical and effective palliative care. 9. Assesses eligibility for palliative care and hospice services. 10. Exhibits behaviors that reflect commitment to the philosophy and vision of Alive Hospice and Alive Palliative Care. 11. Participates in team meetings as required. 12. Potential to serve as a mentor and preceptor to nursing students, medical students, medical residents and fellows. 13. Assists in orienting new providers to the Alive Palliative Care team. 14. Maintains state and national certification (discipline specific) and demonstrates continued competency in the management of patient care 15. Promotes excellence in patient care. 16. Participates in required continuing education to maintain credentialing and compliance for state licensure and national certification. Remains current on research findings and integrates into practice. 17. Other duties may also be assigned.
Requirements:
EDUCATION and/or
EXPERIENCE
Graduation from an accredited APP degree program (ex:
NP, CNS, PA
); Masters level degree preferred. Experience in hospice or palliative care preferred. Experience in ICU or inpatient acute care preferred.
CERTIFICATES, LICENSES, REGISTRATIONS
Current/Active and unrestricted Tennessee licensure, and maintenance of national certification in associated discipline (ex:
NP/CNS/PA
) along with ability to maintain unrestricted DEA for prescribing practices. If required to drive to carry out the duties of this position: current driver’s license and automobile insurance as required by Tennessee State Law.
