Job Description Summary:
The Palliative Care Nurse Practitioner conducts comprehensive health assessments for adult patients with life threatening or life debilitating illness. Works under established supervision using the APP Palliative Care Service Protocol. Models appropriate behavior to employees as exemplified in the Quality Plus manner, which reflects commitment to the Continuous Process Improvement Plan (CPI).
ESSENTIAL JOB FUNCTIONS
Collaborates with the supervising physician in establishing/ reviewing protocols for managing acute and long-term medical needs of the patients.
Performs a complete physical exam, obtain a complete medical history and records findings. Orders appropriate laboratory and diagnostic procedures. Interprets and integrates data to determine preliminary diagnoses and therapeutic plan.
Provides medical services and health education to adult and geriatric patients under the supervision of the APP’s supervising physician.
Performs necessary diagnostic and therapeutic procedures according to approved protocols developed with and under the palliative care physician supervision which may include the following: securing, recording and evaluating the health, psychosocial and developmental history of patients, ordering laboratory and x-ray studies, carrying out prescription drug orders under protocols, management of common or chronic illnesses, referrals, etc.
Administers or gives orders to nursing support to administer medications and injections in compliance with state law and Protocol.
Functions as a patient advocate, conducts follow-up visits and coordinates care with other health care providers and makes appropriate referrals for continuity of care.
Maintains accurate and complete medical records in a timely fashion.
Demonstrates effective guest excellence skills with patients, families and guest.
Provide educational counseling that will enhance patient care and preventative knowledge.
Contributes to a positive work climate and the overall team effort of the department.
Performs related responsibilities as required or directed.
JOB SPECIFICATIONS
EDUCATION:
Master’s or Doctorate Degree in Nursing or a Physician’s Assistant Degree with successful completion of an accredited advanced practice program approved by the state of Tennessee.
LICENSURE, REGISTRATION, CERTIFICATION
Must possess unencumbered license as an Advance Practice Nurse in state of Tennessee.
Must be certified by the American Association of nurse Practitioner (AANP) or American Nurse Credentialing Center (ANCC)
Must have an NPI number.
Must have or applied for DEA number.
Would prefer an Advanced Certified Palliative and Hospice Nurse (ACPHN) or actively pursuing.
Valid TN driver’s license
EXPERIENCE
Prefer two years experience as a Family Nurse Practitioner
Prefer Palliative/Hospice background.
KNOWLEDGE, SKILLS AND ABILITIES
Knowledge of nurse practitioner principles and techniques. Demonstrate the ability to work cooperatively and productively in a team environment with a diverse set of employees, patients and families.
Possesses computer skills sufficient to conduct word processing for document development and patient record documentation.
