3 – 4-day work week with shorter day-to-day hours at the Nursing Home Facility and daily call schedule
$55,000 – $65,000 a year with a Base Salary and Compensation Model for Quality & Performance
Summary:
Under the general supervision of the Director, Medical Services, the Advanced Practice Registered Nurse (APRN) participates as an active leader of the Skilled Nursing Facility (SNF) care team. The APRN works autonomously and in collaboration with the facility nursing and management staff, facility medical director, and the Exceptional Care Services (ECS) provider team to provide exceptional care to skilled and long-term care residents.
The Nurse Practitioner is responsible for the management of chronic health conditions and is expected to provide evidence-based care to all residents within the facility.
Exceptional candidate will:
Maintain open communications with patients, families and/or responsible parties, facility staff and attending physicians.
Maintain a customer-oriented approach to meeting patient needs in an expedient manner, involving others in the plan of care as appropriate.
Maintain confidentiality and uphold the highest level of professionalism.
Communicate at all levels of the organization and work well within a team environment in support of company objectives.

Job Requirements:
Successful completion of an approved Nurse Practitioner program.
Master’s degree in nursing (MSN) and valid license to legally practice as a nurse practitioner in the state of current practice.
Must have a current/active CPR certification.
Active prescriptive authority and DEA licensure.
Two or more years of clinical experience as a nurse or nurse practitioner at a nursing home facility preferred.
Effective verbal and written English communication skills.
Demonstrated intermediate skills in Microsoft Word, Excel, PowerPoint, Outlook, Internet, and Intranet navigations.
Demonstrated skills in Electronic Medical Records.
Approachable, flexible, and adaptable to change.
