Nurse Practitioner – Psychiatry 3.6 3.6 out of 5 stars Essex Junction, VT 05452 CHC is seeking licensed Psychiatric Nurse Practitioners to join our team! The Advance Practice Practitioner – Psychiatry works directly with at-risk populations and is responsible for providing direct outpatient mental health services and consultation in collaboration with a team of Medical Providers, Clinical Social Workers and with the remainder of the psychiatric team for patients that seek medical services at our Safe Harbor Homeless Healthcare Clinic and our Pearl Street Youth Clinic. Coordinates and documents care for quality and continuity of care, and for risk management. Serves as a member of health care team and assumes responsibility for quality assurance and quality improvement efforts. Basic Qualifications Education Graduation from program of study accredited by the appropriate professional board Preferably 3 or more years of experience in clinical practice Preferably experience with adolescent and transitional age populations Knowledge, Skills, and Abilities Ability to work independently as appropriate to level of training/experience Ability to work collaboratively with multiple disciplines to support excellent patient care Ability to problem solve and collaborate in the care of individuals complicated by insecure housing and other significant social determinants of health and mental health Ability to solve problems creatively and with initiative Ability to work flexible hours Licensure, Certification, or Other Requirements Board certified or eligible for board certification For Physician Assistants, must have CAQ in Psychiatry CPR certification Licensure for practice in the State of Vermont Completion of requirements for CHC practitioner credentialing Compensation commensurate with direct experience within the range of $107,782 – $131,734 base pay annually.
