A Brief Overview Accountable for providing direct, personalcare and services to residents in the skilled nursing facility,which meet the physical, academical, social and spiritual needs ona daily basis to maintain and improve their highest level ofindependent function. What you will do Provides assistance toresidents with their activities of daily living Performs routineduties and non-routine duties as directed by nursing staff. Thismay include but not limited to assisting patients with bathing anddressing needs, shaving, oral hygiene, bed making, transfers intoand out of bed, repositioning, toileting needs, serving andcollecting food trays, encouraging and/or assisting with feeding,answering call lights, insuring a clean environment, transportingpatients upon admission and dismissal, assisting with ambulationand orienting/reorienting of resident to the facility environmentto insure resident safety. Performs blood pressure, pulse,temperature, respiration and weight. Documents concise and factualinformation on the appropriate forms. Performs or assists residentsin performing specific medical care activities in accordance withestablished policy and procedure to contribute to the patient’splan of care. This includes assisting residents in performingrestorative exercises, collecting specimens as directed by nursingstaff. Assessing and documenting completed cares. Provide feedbackto nursing staff immediately on status or changes in patientconditions, pain management, and other important concerns. Assistswith restocking supplies and maintaining appropriate workenvironment in unit including cleaning procedures. Participates inproviding emotional, educational and spiritual support forfamilies. Complies with all aspects of resident rights includingconfidentiality and privacy. Reports all allegations of residentabuse or misappropriation of resident property. Understanding andfollowing established policy and procedures related to fire,tornado, evacuation, disaster plan, universal precautions,infection control procedures, hazardous and chemical communicationprogram. Maintains an environment that is clean and safe for allresidents, families and staff. Essential Qualifications Theindividual must be able to work the hours specified. To performthis job successfully, an individual must be able to perform eachessential job function satisfactorily including having visualacuity adequate to perform position duties and the ability tocommunicate effectively with others, hear, understand anddistinguish speech and other sounds.
