A Brief Overview Accountable for preparing, administering and documenting medicationadministration in accordance with established policy and procedureunder the supervision of a licensed nurse. Provides age appropriatepersonal care to residents and maintains a sanitary, orderly andattractive environment. What you will do Provides assistance to residents with their activities of dailyliving. Performs routine duties and non-routine duties as directed bynursing staff. This may includes but not limited to assistingpatients with bathing and dressing needs, shaving, oral hygiene,bed making, transfers into and out of bed, repositioning, toiletingneeds, serving and collecting food trays, encouraging and/orassisting with feeding, answering call lights, insuring a cleanenvironment, transporting patients upon admission and dismissal,assisting with ambulation and orienting/reorienting of resident tothe facility environment to insure resident safety. Performs blood pressure, pulse, temperature, respiration andweight. Documents concise and factual information on theappropriate forms. Performs or assists residents in performing specific medicalcare activities in accordance with established policy and procedureto contribute to the patient’s plan of care. This includesassisting residents in performing restorative exercises, collectingspecimens as directed by nursing staff. Assessing and documentingcompleted cares. Provide feedback to nursing staff immediately onstatus or changes in patient conditions, pain management, and otherimportant concerns. Assists with restocking supplies and maintaining appropriatework environment in unit including cleaning procedures. Participates in providing emotional and spiritual support forfamilies. Complies with all aspects of resident rights includingconfidentiality and privacy. Report all allegations of residentabuse or misappropriation of resident property. Understanding and following established policy and proceduresrelated to fire, tornado, evacuation, disaster plan, universalprecautions, infection control procedures, hazardous and chemicalcommunication program. Maintains an environment that is clean andsafe for all residents, families and staff. Prepares, administers and documents medication administrationin accordance with established policy and procedure under thesupervision of a licensed nurse. Documents reasons prescibedmedication are not administered and reports this to the chargenurse. Essential Qualifications The individual must be able to work the hours specified. To performthis job successfully, an individual must be able to perform eachessential job function satisfactorily including having visualacuity adequate to perform position duties and the ability tocommunicate effectively with others, hear, understand anddistinguish speech and other sounds. Required Education, License/Certification, or Work Experience:

High School or GED Equivalent Certified Nurse Aide (CNA) – Licensing Board An active licensein the state of practice within 120 Days and Medication Aide – Licensing Board An active license in thestate of practice within 120

Days Preferred Education, License/Certification, or Work Experience:

Less than 1 year Experience in Long Term Care, housekeeping,dietary, laundry or activities.
