SummaryUnder the supervision of an RN or LPN,performs basic patient/resident careactivities.

ResponsibilitiesPerforms basic patient/resident careactivities as delegated and supervised by an RN or LPN, includingmonitoring vital signs, taking blood glucose measurements,performing basic phlebotomy, and recording intake andoutput.

Assists with keeping patient rooms stocked, clean, andorderly.

Assists patients with tending to personal care andactivities of daily living, including bathing, grooming andeating.

Reports abnormal assessment findings or changes in physical,mental and emotional conditions to nursingstaff.
