JOB SUMMARY :

Responsible for providing compassionate personal care and homemaker services for patients when appropriate and maintaining a safe and healthy environment as directed by the IDT plan of care.

QUALIFICATIONS

High school diploma or equivalent Current Certified Georgia Nurse’s Aide certificate Current CPR certification Valid driver’s license, personal vehicle for on-the-job transportation, and evidence of insurance coverage as required by company policy. Ability to communicate well, both verbally and in writing Required to have visual acuity to operate motor vehicles.

JOB CONDITIONS AND WORK DEMANDS

Requires minimal lifting of 50 lbs occasionally and up to 10 pounds of force constantly to move objects. Prolonged periods of sitting with periodic kneeling, standing, stooping, pushing, pulling and walking, as well as communication by phone and in person. Travel will be required, by car. Both inside and outside environmental conditions. Work Monday through Friday may include on-call work weekends, evenings, and nights.

RESPONSIBILITIES

: (Included but not limited to): Provide quality care to residents in an environment which promotes their rights, dignity, freedom of choice, and their individuality. Follow the written Plan of Care as directed by the RN. Observe, report, and document of patient status and the care or service furnished. Observe deviations from the usual physical and mental conditions, report to Primary RN or RN Supervisor. Communicate with and support patient and family. Read and record temperature, pulse, and respiration as directed by the plan of care. Utilize basic infection control procedures. Recognize basic elements of body functioning and changes in body function that must be reported to an aide’s supervisor. Maintain a clean, safe, and healthy patient environment. Change bed linens as directed by the plan of care Recognize emergencies and utilize knowledge of emergency procedures. Show respect for the patient, the patient ‘ s privacy, and the patient ‘ s property. Utilize appropriate and safe techniques in personal hygiene and grooming that include bed bath; sponge, tub, or shower bath; shampooing in the sink, tub, or bed; shaving, nail and skin care; oral hygiene; and toileting and elimination as directed by the plan of care. Utilize Safe transfer techniques and ambulation as directed by the plan of care. Aide in Normal range of motion and positioning after demonstration by RN or therapist; as directed by the plan of care. Monitor and report nutrition and fluid intake as directed by the plan of care. Prepare and assist with eating as directed by the plan of care. Any other task that the hospice may choose to have the personal care aide perform, as authorized by law. Document visit record electronically within 24 hours of the visit. Maintain and respect the confidentiality of medical records and information concerning patients. Responsible for submitting patient charges, mileage, and supplies to office staff. Participate in IDT meetings as directed. Provide care in all areas Family hospice covers on assigned office counties. Cover weekend/holiday schedule as assigned. Perform other duties as assigned.
