Responsibilities Reports any unusual circumstance in the patient’s condition orhome environment to the primary RN/Patient Care Manager. Follows current written aide assignment sheet to providepersonal care and assistance with activities of daily living (ADLs)to include mobility transfer, walking, grooming, bathing, dressingor undressing, eating, or toileting. As assigned, performs incidental household services essentialto the patient’s health care at home that are necessary to preventor postpone institutionalization. Completes a clinical note for each visit within requiredtimeframes, which must be incorporated into the patient’srecord.

Education And ExperienceLicense Requirements Successful completion of a competency evaluation. Current CPR certification required. Current driver’s license, vehicle insurance, and access to adependable vehicle Must have a minimum of one-year full-time experience in directclient care Must be mature and able to deal effectively with the demands ofa home health aide position. Able to read, write, and carry outdirections promptly and accurately.
