Hospice Aide (HHA/CNA) Position Available In Berkshire, Massachusetts

The Hospice Aide is a trained support member of the interdisciplinaryteam with responsibility for specialized services, which provide comfortand personal care to hospice patients. The scope of work assigned to aHospice Aide is determined by the individual interdisciplinary plan ofcare and will be in accordance with all agency policies and regulationsthat govern the provision of the service.

• EssentialJob Functions
• Provides direct patient care within the parameters ofcurrent CNA license and in accordance with Hospice Aide competencychecklist.
• Assists in providing personal care services per plan ofcare.
• Uses and maintains special equipment and suppliesappropriately
• Informs supervisor/RN case manager of any changes inpatient’s condition or family situation.
• Recognizes need forguidance, accepts and or seeks assistance.
• Accompanies patient outsidethe home when directed.
• Reports concerns of patient abuse or neglectimmediately to RN/case manager/supervisor.
• Maintains a clean and safeenvironment for patient by performing household services that promotethe comfort, safety, and hygiene of the patient per plan of care.
• Assists in meeting the nutritional needs and dietary requirements of thepatient by performing the following per plan of care: meal preparation,offering fluids and nutrition, assisting with feedings.
• A HospiceAide, with additional training and proven performance, may assist withother certain procedures per plan of care such as: applying certainprescription creams and ointments, assisting with care of colostomy andappliances, or assisting with use of special devices such as a HoyerLift.
• Qualifications
• :•GED or better.
• CNA (Certified NursingAssistant) or HHA (Home Health Aide) Certification

• ExperienceRequired:

• 1 year CNA experience
• Home Health Aide or Hospiceexperience preferred.
