4/23/25 Job Summary Serves as a resource for the nursing/health care team. Responsible for RN/Route Nurse position description. Coordinates the daily schedule for the team. Essential Duties and Responsibilities +

Professional Responsibilities:

+ Coordinate the daily schedule for the team. + Communicate effectively and appropriately between all health team members. + Attend all in-services, education programs and scheduled meetings as outlined by the Director of Nursing/Home Health and Hospice Director. + Consistently demonstrate proper nursing judgment, deliver patient care within the scope of service for RN. + Maintain and demonstrate constant awareness of State and Federal Regulations pertaining to Home Health care. +

Core Duties:

+ Demonstrate performance standards + Develop staffing assignments adjusting as needed + Supervise all aspects of patient care to ensure quality patient care and safety for the team + Scheduled PRN personnel for overflow between teams and communicate patient needs; assist with updating weekend and night call schedules + Schedule SAV’s and provide supervision for nurse aide’ complete and document supervised aide visit on appropriate form + Report patient changes and problems to physician, Director and other health team members + Communicates with RN/Clinical Liaison daily and as appropriate concerning patient care. + Document accurately and complete according to agency policy and procedure + Document interim orders and case conferences with routing to appropriate team members, patient and family; update patient chart, call box and PRN files + Provide expertise to health care personnel regarding treatments, diagnostic procedures, preventative and rehabilitative nursing procedures + Assist in evaluation process for RN and Nurse Aides Qualifications Education and Experience + Graduate of an accredited school of nursing + Current Mississippi RN license + CPR certification +

Phlebotomy:

Establish IV access + Safe blood administration + Medication monitoring + Preventive skin care + Chest tube care + Aseptic wound care/ dressing change + Foley catheter insertion/care Working Conditions Ability to sit, bend, stoop, kneel, crouch, perform overhead lifting and perform other common physical movements as needed for the position.
