PRN Support multiplelocations within 150 miles of your permanent address ~ Mileagereimbursement ~ Additional $5 per hour float premium ~ 25 per hourweekend shift differential ~ 25 per hour evening/night shiftdifferential; Required to work at least one weekend shift per month~3-month previous experience in Long Term Care required, 6 monthspreferred The Nursing Assistant (NA) serves as caregiver to theresident during the scheduled work period in long term care.

Provides resident-centered nursing care and daily living assistanceto assigned resident under the supervision of a registered nurse(RN). Knowledge of and delivers age-appropriate care related to thephysical and psychological needs of the resident as per care plan.

Considered a member of the nursing team and is expected to know,and will be held accountable for, following infection preventionand control policies and Personal Protective Equipment use.

Provides assistance with basic health care needs including dailyliving activities that may include, but are not limited to,bathing, toileting, grooming, dressing/undressing, obtaining andrecording vital signs, and providing psychosocial support and otherpersonal care to assigned resident. Assists the resident intransferring, repositioning, and walking using correct andappropriate transfer techniques and equipment and also providesrange of motion and passive exercises. Completes tasks related tofood preparation and dining, while creating a positive diningexperience for the resident. Assists residents with meals andsnacks, provides water and supplements as appropriate. The NA aidsproviders and nursing staff members with procedures, if needed.

Documents resident interactions as needed and the outcomes relatedto resident care, behavior, activity, and dietary. Communicatesresident’s changing condition and care related concerns/responsesto the charge nurse. Provides housekeeping and laundry services toassist residents with keeping living areas clean and operational.

Provides assistance and documentation with wellness andrecreational programs and encourages residents to participate. Highschool graduate or general equivalency diploma (GED) preferred, butnot required. Prior clinical or nursing assistant experience inlong-term care preferred. Required Certified Nursing Assistant(CNA) certification with the State Board of Nursing, or statecertified CNA within four months of date into the position by beingenrolled in a training program or in the process of obtainingcertification through Endorsement. CNA training and certificationrequired and provided by facility. Depending on facility, QualifiedMedication Administration Person (QMAP) certification with theColorado Department of Public Health & Environment required.

Satisfactorily completes yearly department specific competencytesting and maintains a record of continuing education credits foreach year. Dependent on facility and required responsibilities,this role may classify as a Sanford Category II Driver requiring amotor vehicle report and proof of valid driver’s license. Also mustbe medically qualified to perform the essential driving functionsof this position as per our Sanford Driving Policy per the leadersrequest.
