Full Time. Hospice Team Lead- Rocky Mount Region. Companyequipment, mileage reimbursement.

Serves as a team leader, clinicalresource person, role model, mentor, and facilitator for staff anddepartment operations. Is responsible and accountable to leadstaff; resolve routine issues, and create a positive, productive,and fiscally responsible team. Remains aware of organizationaldevelopments and initiatives and implements as needed for theirteam. Refers complex or potentially significant issues tomanager/supervisor, including but not limited to; issues regardingcompensation, work hours, working conditions, hiring, and formalcorrective action. Initiates and facilitates effective open two-waycommunication and resolution of work site operational issues thatsupport service area goals and objectives. In addition to the leadresponsibilities, maintains clinical competencies and providesprofessional care to patients of all ages in response to agencyneeds.

Assesses, plans, implements, and evaluates patient care inorder to achieve optimum patient outcomes for patients asappropriate to service area.

Utilizes leadership principles inmobilizing the Carilion Home Care/Hospice team to achieve positivepatient outcomes. Follows-up on matters to ensure they aresuccessfully resolved.

Assists manager/supervisor in assuring staffcompetence and contributes to staff performance development.

Identifies areas of improvement.

Assists with the coordination ofwork flow process of the home care/hospice team.

Collaborates withmanager/supervisor regarding fiscal management. Monitors staffproductivity overtime, mileage, and supply usage to ensureappropriate resource allocation and expense control.

Assists withother agency responsibilities including but not limited to;implementing and managing daily activities of the office,maintaining regulatory compliance, reviewing documentation, ICD-10coding, monitoring of face-to-face documents, re-certifications,participating in performance improvement and other qualityimprovement initiatives. Ensures Joint Commission, Medicare andother third party payer guidelines are implemented and followed.

Ensures compliance with policies and procedures.

Conducts individualcase conferences and participates in interdisciplinary teammeetings to facilitate effective two way communications. Listens tostaff members needs and concerns and resolves items brought totheir attention. As appropriate, follows up with staff members onidentified issues and concerns and reports to manager/supervisor asneeded. When indicated, Initiates corrective action policy byemployee coaching. Promotes positive work environment by holdingstaff accountable for their conduct, while using effectiveinterpersonal skills. Demonstrates and encourages SBARcommunication. Performs other duties as assigned.

Continuingeducation guidelines support the concept that learning is acontinual process. For the Clinical Team Leader (CTL) job positionthe minimum requirement of continuing education is 20 hoursannually.

Participates in special hospice events (e.g. grief camps,memorial services, etc.)

What We Require:
Education:

Graduate ofaccredited Registered Nurse program. Bachelors degreepreferred.

Experience:

Five years of clinical experience required.

Demonstrated strong leadership skills, supervisory experiencepreferred. Home Health/Hospice experience preferred.

Licensure,certification, and/or registration: Currently licensed asRegistered Nurse in the state of Virginia or approved compactstate. Must have valid driver’s license, automobile insurance, andpersonal vehicle for making home visits.

Life Support:

Mustobtain/maintain AHA BLS-HCP.

Other Minimum Qualifications:

Demonstrates leadership qualities at current level of practice,good interpersonal and communication skills, and teambuildingskills required. Must successfully complete orientation andcompetency requirements. Keyboarding skills required for automatedcomputer systems, including electronic medical records. Must beable to meet essential job functions.
