Register Nurse SupervisorJob SummaryAssist individuals and families to remain as independent as possible, and to avoid unnecessary or premature institutionalization by providing personal care, light housekeeping, and meal preparation in the home setting.
Key Working RelationshipsThe LNA reports to the Registered Nurse Supervisor
QUALIFICATIONS
- Education
- High school diploma or equivalentSuccessful completion of a Licensed/Certified Nursing Assistant training program and competency testing
- Licenses/Insurance
- Current valid license as a Licensed/certified nursing assistant in New HampshireAccess to operable, insured motor vehicle and valid drivers license with acceptable record of safe driving practice.

ESSENTIAL FUNCTIONS
- Performs or assists clients with personal care including bath or shower, hair, skin, mouth, and nail care
- Assists clients with elimination using bathroom, commode, bedpan, or urinal
- Assists clients with dressing or undressing
- Monitors and records vital signs
- Assists clients with prescribed exercises after instruction by the RN, Physical Therapist or Occupational Therapist
- Assists clients with ambulation and transfer activities
- Assists clients with simple dressing changes after instruction by RN
- Participates in ongoing implementation of client care plans
- Reports pertinent observations of clients physical and psychosocial status
- Documents accurately all care and observations
- Submits accurate and timely reports of client visit charges, hours of work, and mileage
- Practices proper body mechanics and observes appropriate safety measures
- Demonstrates, uses, and maintains client and agency equipment, as instructed. Consistently demonstrates compliance in delivery, charging and utilization of client supplies
- Reports all employee and/or client accidents/occurrences to Supervisor immediately
- Adheres to daily client schedule and accepts assignment changes in a positive manner
- Demonstrates flexibility, reliability, and punctuality with consideration of scheduled assignments
- Attend meetings, seminars, and workshops applicable to this position to enhance skills and job performance
- Preservation of confidentiality of client and Agency information is mandatory.

SPECIAL FUNCTIONS/RESPONSIBILITIES
(Non-essential, but possible):
- The Agency Staff Nurse, Physical Therapist, or Speech Therapist may teach and closely supervise the home health aide in the following procedures
- Apply prescribed ice cap or ice collar
- Assist with prescribed exercises taught to the home health aide by appropriate professional staff
- Uncomplicated dressing changes

LIMITED OR RESTRICTED FUNCTIONS/RESPONSIBILITIES
- The following functions and responsibilities will NOT be provided by the Home Health Aide
- SSE enemas
- Colostomy irrigations
- Gastric lavage or gavage
- Application of heat treatment
- Giving medication in any form
- Giving injections
- Giving advice on medical or nursing problems
- Catherization
- Transporting client or family
- Acting as a guardian or Power of Attorney for client
- Contracting in a private duty capacity with a current and active client of the Agency

CLERICAL FUNCTIONS/RESPONSIBILITIES
- Document daily home visits
- Complete daily time sheets
- Utilizes Documentation System
- Keep records in accordance with agency policy

CONDITION OF WORK
SETTING and
ACCESS TO CLIENT HOME
- Since it is the purpose and nature of the agencys function to work in clients homes, employees are required to understand and accept the condition(s) which may be presented in each individuals residence.
The agency will not permit employees to work in unsafe settings; however, they may be required to work in settings that do not meet the r own personal standards. Furthermore, the agency is unable to control the access or location of a clients home. Employees are to be prepared with proper clothing to drive and walk to a clients home. If a clients home is unsafe or a threat to public and personal health, the employee (in collaboration with their supervisor) shall report their findings to the appropriate enforcement agency. The agency reserves the right not to send employees into settings that the agency determines are unsafe and unhealthy.

ENVIRONMENTAL CONDITIONS
- Work could be in varied environmental conditions, may not always be climate controlled
- May be exposed to second hand smoke, animals, and unsanitary household factors
- May be subjected to emotionally upset clients and family members
- This position involves exposure to fumes, dust, humidity, cold, heat, blood, body fluids or tissue

- All agency employees are required to follow the policies and procedures regarding exposure/infection control, including Universal and Standard Precautions and all required trainings and educational programs.

CONFIDENTIALITY
- Protecting the integrity and confidentiality of the clients personal and medical history, as well as their records, is of major importance to the agency. An employee is not to discuss the medical condition of any client without prior written authorization from the client
- The agency Release of Information statement restricts all employees of the agency to discuss medical issues pertinent to a client or a clients family only with those individuals with a need to know (i.e. physician, supervisor, co-nurse or co-professional assigned to case, immediate family), and only with proper authorization
- Any breach of client confidentiality will result in discipline and may result in immediate dismissal.
