Job:

Performs delegated nursing,homemaking, and other delegated duties for patient/clients of allages and families in their home or other designated facility. Mayperform PT/INR checks in the home as ordered by the Physician andreports results according to policy.

What You’ll Do:

Reports and documents patient/client’s needs and responses tothe appropriate Case Manager and/or Clinical Manager. Reports patient/client’s change in condition or situationsimmediately. Adheres to patient/client Home Health Aide care plan as set upthe primary nurse/therapist while maintaining a healthy, safeenvironment. Reinforces the teaching of the primary nurse/therapist byreviewing personal care techniques with caregivers andpatients. Completes documentation in clinical records, time sheets andquality control logs. Attends a minimum of 12 hours of in-services per year. Provides care while promoting client/patient dignity andprovides a calm, supportive and therapeutic environment forclient/patients and their families.

Who You Are:

Education:

-Highschool diploma or equivalent preferred.

Experience:

Six (6) monthsto one (1) year of experience in a related fielddesired.

Certification/Licensure:

Current Nursing Assistant Licensein the State of New Hampshire required.

Schedule:

-Work schedule mayinclude 8 to 12 hours per day with weekend, evening, night, andon-call coverage based on VNA needs. Staff are expected to coverfor absences by rotating or working extra time. Staff are assignedto the service area of the VNA to meet patient/client needs.

Other:

-Driver’s License required to travel throughout New Hampshire toprovide home health care service and other job-related activities.

Must maintain automobile insurance coverage at established EHSminimum at all times of employment.
