SUMMARY:

As a Certified NursingAssistant, you will provide high quality, individualized patientcare in collaboration with the nursing team. Your ability to takeinitiative while always assisting and maintaining patient care willbe critical in the success of the entire team.

RESPONSIBILITIES

• Patient Care & Service
• Provide and/or assist the resident inactivities necessary as per individualized care planning; transportto and from activities as needed; provide leisure activities asneeded; perform evaluation and reporting; provide resident andfamily with emotional support
• Safety
• Assist in maintaining aclean and safe resident environment
• Communication & Documentation
• Support families, direct them to appropriate professional staff;respond to resident requests in a timely manner; receive/givereports to team members/team leader

REQUIRED QUALIFICATIONS

• Currently certified in New York State as a Certified NursingAssistant.

PREFERRED QUALIFICATIONS

• High School Diploma orequivalent preferred.
• Strong customer service and communicationskills preferred.
